We report a 58-year-old woman patient with jaundice and weight loss. Abdominal computed tomography, magnetic resonance cholangiography and endoscopic retrograde cholangiography revealed a mass at ampulla of Vater with regional lymph nodes enlargement. Contrast enhanced endoscopic ultrasound revealed well-demarcated hypovascular mass, and the mass was not enhanced. Endoscopic biopsy was performed at ampullary mass, which was covered with normal mucosa. Ulcerative lesion was found on orifice. The patient had pylorus-preserving pancreaticoduodenectomy. On pathological examination of the resected specimen, the mass of ampulla of Vater was small cell carcinoma, and immunohistochemisty showed positive reaction for CD56 (cluster of differentiation 56) and synaptophysin.
